How to Generate Assessment Center Cover Sheets

By following the instructions below, you will be able to complete your Assessment Center Cover Sheets.

Starting Point

  1. Log into Star Port using your TUID.
  2. Click on the Faculty tab. In the box entitled Assessment Center - Faculty Information, click on Test Cover Sheets Form. This will take you to the Test Referral Form for make up and in class exercise form.

Section 1: Instructor, Course, and Student Information

  1. On the referral form, select whether the test is Make-up (face-to-face), Online, or Hybrid. (This releases the course list according to the course type. If this is left unchecked, you will not see your course in the list.)
  2. Your name, email, and on campus phone number should already appear in their respective fields. If not, please enter them now.
  3. Under Term, select the term for which the test is required from the pull down menu. You may have to wait for the page to refresh, and this may take a few moments.
  4. Under Location, the Assessment Center will be the default. Select the location where the test will be proctored.
  5. Under Course, select the course name and CRN from the pull down menu. You may have to wait for the page to refresh. (In order to use the class list function, you must have already selected a term.)
  6. Under Students, you can identify the student(s) you need a cover sheet for by one of the following methods:
    • To identify a single student, use the small up and down arrows to locate and then highlight, by mouse clicking once, the name of the student.
    • To identify all students or several non-contiguous students, click on the icon to the right of the field. The student list with check boxes will appear. Check the students participation, then click the submit button.
  7. Under Test ID, give the test a name or enter a description. Use a simple name for your test, for example: Test 5 or Midterm. (AVOID using any special characters, such as &,*,?,%,@,?, as these may cause some problems.)
  8. Under Time Limit (Max 4hrs), set a time limit for taking the test. If you choose to give a time limit, you must do so using time markings (example: 2 hours or 2.5 hours). If you choose not to give a time limit, please type none.
  9. Under Date for Completion, enter the last date that the student is allowed to take the test (you may use the calendar link.)
    (Please note: The Blank Form button at the top left of the screen, allows for the printing of a blank form should you want to complete one or more cover sheets by hand. This button takes you directly to Step 16 below.)
  10. Click the Section 2 at the top right of the screen.

Section 2: Materials and Special Instructions

  1. Select the materials that you wish to be used with the test.
    • No Materials-The student will not be allowed to use any materials during the test.
    • Other Materials Allowed-The student will be allowed only the materials that you have checked off.
  2. Under Special Instructions, you may type in any other specific instructions for the student(s) taking the test. (AVOID using any special characters such as &,*,?,%,@,?, as these may cause some problems.)
  3. Click on the Generate Forms button at the top right of the screen.

Section 3: View or Send Forms

  1. You will see a window that counts down for two minutes. You must WAIT for this time to expire before you can proceed. When the count reaches zero, click the Close Window button.
  2. After the cover sheet(s) have been generated, you have the following options:
    • View: Will open a new window for printing cover sheets. Once the window opens with the document preview, use the browser menu and choose File and select Print. When printing more than one cover sheet, you may need to set the margins to accommodate the particular printer you are using.
      • To print to the printer in the CTE, set the bottom margin to .955 to ensure each sheet prints on its own page.
      • To print to other printers you may need to adjust your margins similarly through trial and error to ensure each sheet prints on its own page.
    • Send: The Assessment Center will accept Cover Sheets only via email (lcc_online_testing@lcc.edu).

Please note: Be sure to read the Assessment Center policy on accepting emailed Cover Sheets and check the Agree box.
